Credit Card Authorization Form | ACH Authorization Form

You can edit these forms 2 ways:

  • 1 - Go to Form, Go to File - Download As - Excel File. You can then fill out the form, save, and email to [email protected].
  • 2 - (Only use this option if you are signed in to Google) Go to File - Make a copy. You can then fill out the form, copy the link at the top of the page and email to [email protected].

Easy Order Form - We will send you the widest variety of weights and colors we can.

Lone Star Order Form - We will send you the widest variety of weights and colors we can.

If you have general guidelines you want us to follow (e.g. 'all heavy', 'no pink', etc) please put them in the notes sections.

Item availability is constanlty subject to change. Items that go out of stock will automatically be cancelled from your order.

We will send you the widest variety of weights and colors we can. 

Returns of stock products are subject to a 15% restocking fee in the form of wholesale credit. Authorization is required before returns can be sent.

Discs Unlimited is not responsible for shipping charges of any returned product. Limited Edition, discontinued and custom discs are not eligible for return.

Please note that added insurance is NOT included in your shipment unless you request it. Is a shipment is lost, we must be notified within a week of scheduled delivery to file a claim with UPS. The maximum amount you are eligible to receive is $100 from UPS after a claim is filed and approved from UPS if additional insurance is not requested.
